What is a freak accident? Put simply it is an accident you could never have seen coming. It might be something unusual that has long odds of actually occurring. For example a freak accident at work may have no warning that it is about to occur, giving you no chance of being able to avoid it happening in the first place.
Risk assessments are commonly used in the health and safety field to ensure as few risks as possible are present in the workplace. If an assessment is properly done it should identify all the potential risks and suggest ways in which those risks can be mitigated. However it still may not account for a freak accident at work taking place.

Really there are all kinds of things that could happen that might fall into this category. As such there are all kinds of potential injuries that might be suffered as well.
