Pressure vessel explodes in carpet manufacturers factory
A case recently came to light whereby a pressure vessel, which was part of the plant in a Telford carpet manufacturer’s factory exploded, sending its lid flying several metres into the air, to collide with a steel girder on the ceiling from whence it fell to the floor very nearly crushing an employee.

The explosion came about because the vessel’s regulator and pressure relief valve system failed. This failure was due to lack of regular maintenance. However, lack of maintenance was not the only concern. The periodic statutory examinations that should have been carried out had not been completed for a period of three years.
Carpet manufacturer fined £10,000
When the HSE (Health and Safety Executive) inspector went to site to investigate, they concluded that failure to carry out the required maintenance and inspections contravened Regulation 12 of the Pressures Systems Safety Regulations 2005. The company was fined £10,000 and instructed to pay costs of £1,174.
