    Horrific crush injury

    A 56-year-old agency worker suffered horrific life changing injuries when his hand was crushed in a 160 ton power press incident at a factory in Exhall. He was using his hands to remove pieces of metal that had been pressed, when he inadvertently stepped on the foot pedal activating the machine. The press came down on his hand with devastating results, crushing it so badly he had to have three and a half fingers removed, plus half of his palm.

    Safety system not correctly adjusted

    In the ensuing investigation carried out by the HSE (Health and Safety Executive) it was confirmed that the press had had a new guard fitted. Unfortunately the interlock, which stops the press from working when the guards are not fully closed, had not been correctly set up. It was also discovered that daily checks carried out previously on 12 occasions, had failed to pick up this fault.

    Left disabled for the rest of his life

    The 56-year-old agency worker sustained life changing injuries. He underwent 35 hours worth of surgery. Nonetheless he has been left severely disabled for the rest of his life.

    The company who employed the agency worker entered a guilty plea in respect of two breaches of the Health and Safety at Work Act 1974. They were fined £8,000 and instructed to pay costs amounting to a further £11,300.
