    What is an accident form?

    An accident form may refer to one of the following:

    • A form to fill out in the accident log book at your workplace, if your injury occurred at work
    • An accident form to begin the process of claiming compensation following a personal injury

    If you are using an accident form to make a claim for compensation, it may be lengthy and time consuming, or may it be short and simple. Typically it will take your details including your name, contact information and the area of your body affected by the accident.

    How to claim

    Once you have made initial contact with Accident Advice Helpline, either via our advice line on 0800 689 0500 or 0333 500 0993 or the 30-second calculator, you can relax – the rest is in our hands! We will talk you through every step, and in most cases we can process claims entirely via the telephone. You will likely not need to attend court, although you may be required to attend a medical assessment.
